Bioniz, LLC ("Bioniz") respects the privacy of visitors to its Web sites. This Privacy Policy ("Policy") explains our online information practices regarding the collection of information through our Web sites that can personally identify you and the choices you can make about the way your information is collected and used. Personally identifiable information is information, such as your name, physical address, e-mail address, telephone number, password, username, medical and health information, gender, and age, when it can be linked to you or used to identify you. If you provide us with personally identifiable information, we will treat it in accordance with this Policy. This Policy only covers the Web sites that are registered to Bioniz ("Web sites"), such as www.Bioniz.com. This policy applies whether you are a consumer seeking health information or a health care provider utilizing the Web sites to obtain information or facilitate care to help meet the needs of your patients.
Information Collection and Use
We have designed our Web sites so that you are not required to provide any personally identifiable information unless you are requesting access to certain options and services or you are voluntarily submitting personally identifiable information to us. However, we may collect your personally identifiable information when you are applying for employment with Bioniz online.
We, along with our third party vendors and service providers engaged to provide marketing and advertising and other patient support services on our behalf, only collect personally identifiable information that you voluntarily provide to us. You may choose at any time not to provide personally identifiable information, although this may restrict our ability to provide requested services or information.
The following is personally identifiable information that you may volunteer to provide to us and examples of how we use such information:
Registration: If you complete a registration form on one of our Web sites, including any forms required to submit your personally identifiable information for employment opportunities, we will use the personally identifiable information in such forms to process your request for services and information. We may use such personally identifiable information to communicate with you in writing or by telephone or online, to evaluate your application, to analyze Bioniz staffing needs and practices, or otherwise to efficiently manage the Bioniz hiring process and employee administration. We may combine employment application data with research tools and information from other sources.
Bioniz also measures the performance of its Web sites in order to determine how we can improve and expand upon the content and services we provide. To do this we may collect certain non-personally identifiable information through certain technologies as further described in the Web site Technology section below.
Information Sharing
To respond to your requests or to process your information for any of the activities identified above, we may share your information with our affiliates, business partners, or others who work for us such as service providers, vendors, contractors, and agents. If your personally identifiable information is collected anywhere on the Web sites where there is not an accompanying statement limiting its use to specific purposes or where there is no choice provided regarding the handling of your personally identifiable information, Bioniz may use your personally identifiable information to market any of the products or services offered subject to the "Choice to Opt Out" paragraph below. To the extent you submit personally identifiable information related to a specific initiative or campaign, we may share the information with any subsequently formed affiliate dedicated to such initiative or campaign.
We may partner with other companies to provide you with content or services on a joint or "co-branded" basis. At a co-branded site you would likely see both the Bioniz logo and the logo of the co-branded partner displayed on your screen. To access the services available on a co-branded site you may have to complete a new online registration form, and this registration information may be shared with Bioniz. You should read the individual privacy policies of our co-branded partners, as these may differ in some respects from ours. Reading these policies will help you to make an informed decision about whether to provide your information to a given site. Remember, you may always opt-out of sharing your information with a partner site by choosing not to use the service or content provided by the co-branded site.

Any communication you submit or that may be posted to any publicly viewable interactive feature such as a blog or testimonial page is a public communication and may be viewed by the general public. Therefore, you acknowledge and understand that you have no expectation of privacy or confidentiality in the content you submit to such services over our Web sites, whether or not it includes personally identifiable information about you. If you display your personally identifiable information in any communication submitted to such services or features, other individuals may collect and use your personally identifiable information. While we may remove personally identifiable information from postings to our Web sites, we assume no obligation to do so. Web Site Technology
Non-Personally Identifiable Information: This refers to information that does not, by itself, identify a specific individual. We gather certain information about the use of our Web sites by our visitors. This information is then compiled and analyzed in a way that does not identify our visitors personally. The information may include the referring Uniform Resource Locator (URL), indicating from where a visitor to one or more of our Web sites has come, which URLs have been visited on our Web sites and in what order, the date and time of your visit, cookie preferences, language settings, what browser is being used, and what Internet Protocol (IP) address a visitor used to get to our Web sites. An IP address is a number assigned to your computer whenever you use the Internet. We may also use such information for Web site and system administration. We collect this information through customized software and cookies.
Linked Web Sites: We may provide links to third party Web sites. You acknowledge that such links are provided for your convenience only and do not constitute an endorsement by Bioniz of such third party Web sites. Since we do not control these Web sites, we assume no responsibility for the privacy practices of such Web sites. We encourage you to review the privacy policies posted on those, and all, third party Web sites.
Cookies: Most of our Web sites use cookies, pieces of numeric data that we transfer to be stored on your computer so that we can easily identify your computer when you return to our Web sites. Bioniz can only "read" cookies that are placed on your computer from our own Web sites. Usage of cookies is not linked to any personally identifiable information. You can choose to disable cookies by following the procedure suggested by your particular browser. If you choose to disable cookies in your browser, you may still be able to access portions of our Web site content, but some of the functionality of the Web site may be impaired.
Protecting Your Information
We provide reasonable protections for the personally identifiable information you share with us. We do not knowingly collect personally identifiable information from children under the age of 18. If we learn that we have personally identifiable information on a child under the age of 18 we will delete that information.
Choice to Opt-In
We would like to send you information about our company and our products if you wish to receive such information.
